All-Staff Culture Survey

This page serves as formal confirmation that you have completed the survey.

 Metro Transit employees who are unable to access the survey during their regular shift will be paid for an additional 15 minutes to complete the survey. Use the following instructions to provide this confirmation to your supervisor.

1. You must capture a screenshot or printout of this webpage.

• Screenshot: You may use your smart phone (or someone else’s) to take a photo of the screen. Don’t worry about capturing the full page in your screenshot, any part of the page will work.

• Printout: You may print this webpage if you have access to a printer.

2. Present the screenshot or printout of the confirmation to your supervisor. Your supervisor will be responsible for entering your extra paid time.

3. Please include your name and employee ID if you are sending the screenshot to your supervisor or giving them a printout.
